Report of Absence form is now online

We are excited to announce a new Report of Absence Dynamic Form (e.g. – a web-based electronic form) that will be used by AFO, Local 71, Local 1650, and Exempt employees. This form will replace the current fillable PDF Report of Absence form. The new process eliminates the requirement to scan and/or email copies of absence reports to the Payroll Department.
The Report of Absence form is now available on the Employee Self-Service portal. Here is a direct link to the Report of Absence. (Login is required. To find this link, go to Self-Service, select “Employee,” and you will find the Report of Absence form on the next screen.)
After you complete and submit the electronic form, it will be directly routed to your supervisor. Your supervisor will receive an email that includes a link to your Report of Absence. When your supervisor goes to the link in the email message, they will have the option to sign the form electronically and submit for processing, or return the form to you for revisions.
After the form has been approved, you will receive an email with a link to your completed Report of Absence. The Payroll Department will be automatically notified when a Report of Absence has been approved and submitted, and will record the absence. You are always able to review your leave balances using Self-Service.
This new Dynamic Form eliminates the need to directly send any further PDF reports to the Payroll Department.
Please begin using the new online Report of Absence form for your October absence reports, and all future absence reports.
